Account & Contact Information

When you create an account or contact us, we collect your name, email address, phone number, organization name, and job title.

Platform Usage Data

We collect information about how you interact with Zavio, including log data, device information, IP address, browser type, and pages visited.

Protected Health Information (PHI)

As a Business Associate under HIPAA, Zavio processes PHI on behalf of Covered Entities (our healthcare organization customers). PHI is handled strictly in accordance with our HIPAA Compliance Policy and applicable Business Associate Agreements.

We retain account data for as long as your account is active or as needed to provide services. PHI retention follows the terms of your Business Associate Agreement and applicable law. You may request deletion of your personal data by contacting us.
